The federal Centers for Medicare & Medicaid Services partners with state agencies to inspect nursing homes in Missouri. When facilities violate national standards for quality and safety, they are added to the Special Focus Facility program (SFF). Utilizing a points-based system, state agencies flag SFF homes from a list of candidates with long-term, serious issues.

Current SFF Facilities in Missouri

If your loved one is in an SFF nursing home, know that government agencies have flagged these facilities for repeated violations. Despite this supervision, most people choose to remove family members from the care of these facilities. Below are some of the worst nursing homes in St. Louis, MO, and beyond. 

Liberty Health and Wellness

Inspectors found 107 total deficiencies at Liberty Health and Wellness, located at 2201 Glenn Hendren Dr., Liberty, MO 64068. Eight of these were serious violations related to infection control, abuse, neglect, or exploitation. To date, Liberty Health and Wellness has accrued $480,513 in total fines. The child of one resident states, “This place is not fit for animals.”

Riverside Nursing & Rehabilitation Center, LLC

Located at 4700 NW Cliffview Dr., Riverside, MO 64150, this facility received citations for 51 total deficiencies, one classified as serious. Overmedicating residents and failing to provide adequate supervision contributed to Riverside’s $229,926 in fines. A former employee warns, “There were many unsafe and often illegal practices here in order to save money.”

Levering Regional Health Care Center

With 12 violations, this nursing home at 1734 Market St., Hannibal, MO 63401, has the highest number of serious deficiencies in Missouri. Levering’s 59 total citations included failure to honor resident’s right to dignity, dietary inadequacies, and the presence of abuse, neglect, or exploitation. One family member left a Google review saying, “How it hasn’t been shut down, I don’t understand.”

Using a points-based system, inspectors identify each failure to meet national care requirements.

  2. Green Park Senior Living Community: 128 total deficiencies
  3. Avalon View Health and Wellness: 122 total deficiencies
  4. Hillside Rehab and Healthcare Center: 115 total deficiencies
  5. North Village Park: 108 total deficiencies

Missouri’s SFF Candidate List

Nursing homes on Missouri’s candidate list qualify for SFF status but have not yet been selected by state-level officials. The Centers for Medicare & Medicaid Services requires states to submit five low-ranking candidates for each SFF slot. 

Aspen Point Health and Rehabilitation

Located at 2840 West Clay St., St. Charles, MO 63301, Aspen Point has Missouri’s highest total deficiency count. This one-star, 180-bed nursing home received 131 citations for administrative, quality-of-care, and pharmaceutical deficiencies. In a recent inspection, officials realized that the facility lacked any policy for cleaning. They found trash, unmopped spills, dead insects, dirty clothing, mold, peeling wallpaper, stains, and debris on the floors of residents’ rooms. 

Avalon View Health and Wellness

Describing the neglect her father experienced, one reviewer cautions, “If there is anything you do in this life, please make sure you do not give this facility a dime.” Four residents reported sexual abuse by one of the nurses in January 2022. Avalon View has been cited 122 times and fined $320,077. This one-star home is located at 1200 West College St., Liberty, MO 64068.

Bentleys Extended Care

In their report of 52 total deficiencies, inspectors cited Bentleys at 3060 Ashby Road, Overland, MO 63114, for failing to provide enough food and fluids to maintain residents’ health. This one-star nursing home also neglected to assist with activities of daily living and report suspected abuse.

Country View Nursing Facility, Inc.

Medication errors and unreported falls are just some of the complaints against this nursing home at 2106 W. Main St., Bowling Green, MO 63334. After one resident fell and fractured two bones in May 2023, staff failed to offer proper treatment or contact emergency services. They instead administered one Tylenol, kept the resident on site, and gave them a shower without immobilizing the injured limb. Country View is a 60-bed nursing home with 56 total deficiencies and over $122,000 in fines. 

Dutchtown Care Center

One of the worst nursing homes in St. Louis, Dutchtown Care Center is a nursing facility accused of moving residents without family members’ knowledge. One former employee reviewed the facility and said she wished she could provide a negative number of stars, citing the presence of bed bugs. This one-star center is located at 3421 Gasconade Ave., St. Louis, MO 63118.

The Estates of Perryville, LLC

Based at 430 N. West St., Perryville, MO 63775, this 156-bed facility provides short-term rehabilitation and long-term care. After a cognitively impaired resident drowned in an unlocked bathing room, The Estates of Perryville was cited for failure to supervise them, making it one of the worst nursing homes in Missouri.

Hillside Rehab and Healthcare Center

Found at 1265 McLaran Ave., St. Louis, MO 63147, Hillside Rehab provides long-term and subacute care. During its most recent inspection, officials stated that the facility failed to provide residents with safe, clean housing. Agents found malfunctioning taps, clogged sinks full of standing water, and broken ceiling tiles, among other issues.

Life Care Center of Brookfield

This nursing home, located at 315 Hunt St., Brookfield, MO 64628, received citations for failing to treat six residents with dignity. Staff yelled at one resident and left another exposed and visible to guests while undressed. These incidents contributed to the $179,059 in fines levied against Life Care Center of Brookfield. 

Meadow View Health & Rehabilitation

Meadow View is a one-star nursing and assisted living facility at 2203 E. Mechanic St., Harrisonville, MO 64701. A reviewer states, “Residents sit around in nasty clothes and full diapers. People are very mistreated here.” These conditions have made Meadow View an SFF candidate.

North Village Park

With over 108 deficiencies and $359,365 in total fines, North Village Park is considered one of the worst nursing homes in Missouri. Investigators have documented multiple physical and verbal altercations between staff members and residents at 2041 Silva Lane, Moberly, MO 65270. 

Senath South Health Care Center

Senath South cares for an average of 137 residents per day. The 150-bed facility has failed to protect its residents from unwanted sexual advances, physical attacks, and harassment from other residents at 300 E. Hornbeck St., Senath, MO 63876. Family members have filed complaints about irregular meal times, confiscated snacks, small portion sizes, and other issues contributing to malnutrition.

Baptist Homes, Tri-County

The government has levied over $230,000 in fines against Baptist Homes’ Tri-County location, which can be found at 601 N. Galloway Road, Vandalia, MO 63382. This center has been cited for abuse. Most recently, the nursing facility’s staff failed to address fall risks, which caused one resident to sustain an orbital fracture and a brain bleed. 

Troy Manor

Based at 200 Thompson Dr., Troy, MO 63379, Troy Manor has exhibited failings in pharmaceutical care, nursing services, and care planning. The nursing staff administered the wrong medication to one resident for 97 days. They gave the patient haloperidol, an antipsychotic, instead of metoprolol tartrate, a common drug used to treat high blood pressure. The resident experienced side effects from the unnecessary haloperidol and suffered symptoms usually addressed by their blood pressure medication. This is just one of the 58 deficiencies found at Troy Manor, which also include allegations of abuse.

Windsor Estates of St. Charles

Located at 2150 W. Randolph St., St. Charles, MO 63301, Windsor Estates is a continuum-of-care community that offers services ranging from independent living to memory care. Deficiencies in administration, environment, and quality of life have earned it a place on the SFF candidate list and a one-star rating from Medicare. Recently, the facility was fined over $14,500 for failing to secure a wheelchair-bound resident during bus transport, which resulted in facial fractures and fractured ribs.

Facilities That Have Graduated from the SFF Program

Facilities graduate from the SFF program after sustaining improvement for at least one year. Graduation does not mean that a nursing home has fixed all its issues. It may remain one of the list of worst nursing homes in St. Louis, MO. Instead, this status simply reflects a trend of progress relative to the institution’s prior standing. 

Green Park Senior Living Community

Family members of Green Park residents have made multiple complaints about the community’s odor, upkeep, and understaffing. One woman states, “I am a nurse. I’ve worked long care. I’ve never seen such a poorly run facility.” Although the center graduated from the SFF program in the past, conditions and care remain subpar. To date, the nursing home at 9350 Green Park Road, St. Louis, MO 63123, has received 128 total deficiencies and over $127,000 in total fines. 

Oakwood Estates Nursing & Rehab

Even after graduating from the SFF program, Oakwood Estates continues to receive serious complaints. Family members demand resolution for unpleasant odors, outdated equipment, rodent infestations, aggressive employees, and stolen personal items. One relative writes, “Totally unacceptable. Death toll high. More rats than residents. I wouldn’t send my enemy here. My grandmother beat cancer to die there from aspiration.” Many have called for officials to shut down the facility at 5303 Bermuda Drive, Normandy, MO 63121.

St. John’s Place

St. John’s Place, located at 3333 Brown Road, St. Louis, MO 63114, has a history of 43 total deficiencies and $56,730 in fines. A former employee of St. John’s calls it a “horrible place,” explaining that each nurse oversees more than forty patients on every shift. Family members have documented bed sores, neglect, injuries, frequent altercations with staff, and residents left to sit in their own urine for hours.
