The Levering Regional Health Care Center is a Reliant Care Management affiliate and a large facility with 179 beds. It provides both short-term rehabilitation and long-term care. However, the facility is not part of a continuing care retirement community.
As recently as October 5, 2023, the facility had 59 total deficiencies in care reported against it and has been subject to financial penalties totaling $517,000. These issues have also led to three payment suspensions from the Centers for Medicare and Medicaid Services. Last year, numerous reports brought the Levering Regional Health Care Center’s ability to properly care for residents into serious doubt.

The report cited a failure to keep a sanitary environment for residents by not cleaning walls, floors, toilets, bed linens, and other fixtures after they became soiled with urine and feces. The facility did not replace wet ceiling tiles that contained black mold.

This report indicated a basic failure to provide residents with nutritional and dietary needs, including by not assisting with eating, serving adequate portions, and preparing food according to the facility’s recipes. The facility also failed in health care assessment and planning for residents by not developing care plans and reviewing them with medical professionals.
Standard Report Dated January 4, 2023
The facility’s staff failed to treat residents with dignity and respect in verbal communication, such as by using profanity against them. The report further stated the facility failed to timely investigate and address instances of abuse and neglect toward residents. This included failures to notify doctors and family members about resident injuries and decline in health.
The report further detailed neglect in providing necessary treatments to residents, such as dialysis services and foot care. The facility also failed to keep essential equipment working safely.
