Aspen Point Health and Rehabilitation is a 180-bed for-profit skilled nursing facility. According to ProPublica’s Nursing Home Inspect database, the facility has received $489,000 in federal fines over the past three years for serious care deficiencies.

A review of Aspen Point Health and Rehabilitation complaints and inspection reports paints a picture of ongoing quality of care issues. In December 2022, state investigators cited the facility for 13 health deficiencies ranging in scope and severity from levels C to K. This means inspectors found evidence of actual harm to residents. As a result of this complaint survey, the Centers for Medicare and Medicaid Services fined Aspen Point $150,582 and suspended its Medicare payments. Violations included the following: 

  • The facility failed to ensure one resident received all their physician-ordered medications. The resident, who had a diagnosis of seizures, did not receive their evening and bedtime medications on their admission day and did not receive any medications the following day, including the anti-seizure medication Keppra. As a result, the resident became unresponsive, was sent to the hospital in a postictal state, and had to be emergency intubated. 
  • Facility staff failed to properly identify and treat a deep tissue injury for a resident at risk for pressure ulcers. Upon admission, the resident had multiple suspected deep tissue injuries on the coccyx, heels, feet, and ankle that the facility had not identified or treated.
  • The facility failed to ensure a system was in place for the administrator to know about and pay bills for essential services such as internet and trash disposal. The facility’s internet bill went unpaid, causing a service disruption that prevented staff from accessing electronic medication records and administering medications to at least 27 residents. The administrator did not have access to funds to prevent the disruption.
  • Facility staff failed to immediately inform one resident’s responsible party and physician of the development of skin breakdown. The resident was transferred to an emergency room with pressure sores on the coccyx, heels, feet, and ankle that had not been disclosed to the resident’s family and physician.
  • The facility failed to provide adequate staffing to ensure residents requiring assistance were clean and groomed appropriately. Inspectors observed six residents with poor hygiene and grooming. Staff also failed to complete assessments and documentation for one resident after a fall. Staff reported insufficient staffing or cases in which staff clocked in but failed to complete their assignments.

The U.S. Centers for Medicare & Medicaid Services rates nursing homes in the United States on various factors. Unfortunately, the problems at Aspen Point Health and Rehabilitation in Missouri are not isolated incidents but part of a longstanding pattern, as reflected in government agency reviews of the facility. 

Medicare.gov currently gives Aspen Point its lowest possible overall rating and a “much below average” grade in all subcategories, including health inspections, staffing, and quality measures. Notably, the 71 health citations found at the most recent inspection are more than seven times higher than the U.S. average of 9.3.

  • Health Inspections: 1 out of 5 stars based on 71 citations, vs. 9.3 U.S. average
  • Total Nurse Staffing Hours per Resident per Day: 3 hours 20 minutes, vs. 3 hours 47 minutes national average
  • Registered Nurse Hours per Resident per Day: 19 minutes, vs. 39 minutes national average 
  • Total Nurse Staff Turnover: 100 percent, vs. 52.7 percent national average

ProPublica data further highlights the scope of Aspen Point Health and Rehabilitation’s inadequacies, with inspectors documenting 131 deficiencies in recent years. 

Understaffing appears to be a major issue, with total nurse staffing hours and RN hours per resident falling well below national averages. Meanwhile, the facility lost 100 percent of its nursing staff to turnover within a year. These metrics point to an environment where residents are at risk of neglect.
