What is Elmiron?

Elmiron (pentosan polysulfate sodium) is a popular prescription drug that treats urinary pain caused by interstitial cystitis. It can also be used to treat osteoarthritis. The drug, Elmiron, is manufactured by Janssen Pharmaceuticals, a division of the healthcare giant Johnson & Johnson. Elmiron is classified as a urinary analgesic, or pain reliever, and as an anticoagulant, or blood thinner.

What Is Interstitial Cystitis (IC)?

Interstitial cystitis is a chronic bladder condition that can cause a range of symptoms, including:

  • bladder pressure
  • bladder pain
  • pelvic pain
  • perineum pain

Over one million people in the United States suffer from interstitial cystitis. There is no cure for it, and Elmiron is the only prescription drug that has been approved by the Food & Drug Administration (FDA) to treat interstitial cystitis and painful bladder syndrome.

Elmiron And Vision Loss

The use of Elimron may cause the following common side effects:

  • diarrhea
  • upset stomach
  • abnormal liver functions
  • dizziness
  • bruising
  • nausea
  • blood in the stool
  • headache
  • rash
  • weight gain
  • swelling

The drug manufacturer did not provide any warnings about potential vision impairment, retinal injury, maculopathy, or eye disease. The American Academy of Ophthalmology has issued a report that notes that patients may experience other symptoms such as:

  • vision damage or loss
  • problems with vision focus and reading
  • macula/retinal damage (retinal maculopathy)
  • difficulty adjusting to dim/dark lighting

In 2018 and 2019, researchers found that up to one-quarter (25%) of patients with significant or long-term exposure to Elmiron (pentosan polysulfate sodium) showed demonstrable signs of eye damage. Some evidence may suggest that even when the use of Elmiron is discontinued, vision problems may continue to worsen.

The research in these studies found an increased occurrence of macular degeneration, in particular, an eye disorder called retinal pigmentary maculopathy. The 2019 study conducted at the Emory Eye Center with six patients who had chronic exposure to Elmiron reported that they “had developed unusual changes in their macula.” The 2018 Kaiser Permanente study found that Elmiron “caused retinal damage in about 25% of patients and, adding to the issue, the damage itself can mimic that of other retinal conditions such as age-related macular degeneration or pattern dystrophy.”

Symptoms of Retinal Pigmentary Maculopathy

The symptoms to watch out for are:

  • blurry vision
  • dark spots
  • loss of close vision
  • visual dimming
  • difficulty reading
  • night blindness
  • blindness

Inadequate Warnings For Elmiron Eye Damage

The research also indicated that patients with vision problems might have initially been diagnosed with another vision disorder or eye disease, such as age-related macular degeneration or pattern dystrophy, which have similar symptoms. Researchers have, therefore, suggested that Elmiron patients who notice vision changes or other eye problems should speak with their physician or ophthalmologist about getting an ophthalmic examination with retinal imaging.

Despite the research, no warnings for macular damage, visual changes, or other eye conditions were listed on prescribing information for Elmiron until recently. On June 16, 2020, the FDA approved a label change. Drug manufacturer Janssen Pharmaceuticals admitted that Elmiron, taken over time, “appears to be a risk factor” for side effects including “retinal pigmentary changes.”
